TESTING INFORMATION

The examination for this recruitment involves "successive hurdles." Applicants must successfully complete EACH test phase before proceeding to the next.

2. Physical Ability Test (PAT) (qualifying)

The PAT is a continuous, timed run consisting of the following events:

Event 1: sprint 125 yards;

Event 2: drag a 178-pound dummy a distance of 20 feet;

Event 3: carry a 25-pound backpack a total of 85 feet, including up and down a flight of stairs consisting of 10 steps;

Event 4: push a 138-pound object a distance of 25 feet; and

Event 5: sprint 25 feet to cross the finish line.

These events are designed to simulate job-related tasks Adult Corrections Officer Recruits must perform in providing security and in responding to emergency situations. (Further details will be provided to applicants as they are scheduled to take the PAT.)

3. Oral Interview conducted by the Department of Public Safety.

4. Pre-Employment Assessment Testing.

5. Criminal History Check and Comprehensive Background Investigation: You will be required to sign an Applicant's Consent, Authorization and Request to Release Information Waiver. ADULT CORRECTIONS OFFICER RECRUIT (CO-04) - OAHU Supplemental Questionnaire
* QUESTION 1
BONA FIDE OCCUPATIONAL QUALIFICATION (BFOQ)
Bona Fide Occupational Qualification staffing requirements at each correctional facility with female inmates require that a specified minimum number of correctional officers be female. In the event the total number of female correctional officers at a facility falls below the minimum number required, certification of eligibles shall be limited to female applicants who meet the educational and experience requirements for the ACOR class of work.

The Department of Public Safety (PSD) has informed DHRD that they anticipate ACOR vacancies at correctional facilities statewide which require the incumbents to be female. Therefore, certification of qualified eligibles to these vacancies will be limited to female applicants only.

As we are unable to ascertain applicant's gender by looking at their full names, we ask that you indicate your gender by checking the appropriate box. Your response will not affect your eligibility or opportunity for other ACOR positions.

* QUESTION 2
DRUG FREE REQUIREMENT
Adult Corrections Officer Recruits must be eligible and qualified to use firearms and ammunition at all times. The following question refers to your recent, non medical use of illegal/controlled substances. Do NOT report any medications prescribed by a doctor. Examples of illegal drugs/controlled substances include cocaine; crack cocaine/ice/batu; marijuana; hashish; designer drugs like ecstasy or X; hallucinogens like LSD or PCP; inhalants like paint, glue, or ether; opiates like heroin, opium, or morphine; sedatives like Quaaludes or reds; steroids; amphetamines like speed, uppers, hallucinogenic mushrooms; tranquilizers; valium, poppers or other illegal controlled substances.

* QUESTION 3
SUITABILITY - FEDERAL FIREARM GUN CONTROL ACT - LAUTENBERG AMENDMENT
Convictions of domestic violence preclude incumbents in these positions from carrying firearms and ammunition. Incumbents are required to be qualified to use, carry, and have access to firearms at all times. An amendment to the Federal Firearm Gun Control Act [Title 18, United States Code, Section 922(g)(9)] makes it illegal for anyone who has been convicted of a misdemeanor or felony crime of domestic violence to carry and/or possess any firearm or ammunition. A crime of domestic violence is ANY offense - whether or not specifically identified by law as a crime of domestic violence - which involved the use or attempted use of physical force, or the threatened use of a deadly weapon committed by the victim's current or former spouse, parent or guardian, by a person living or who lived with the victim as a spouse, parent, or guardian, or by a person who has had a similar relationship to the victim. This amendment applies to persons convicted at any time before or after the law was passed. There are no exemptions for law enforcement officers (including correctional officers), government employees or agents. If you are a law enforcement employee and you have a conviction record involving domestic violence, you may be affected by this amendment to the law. In answering the following question, you should check NO if all your convictions have been expunged or set aside or you have been pardoned for all your convictions. However, if any pardon, expungement, or restoration of civil rights expressly provided that you not ship, transport, possess, or receive firearms, you must check YES. You must also answer YES if you received a court martial conviction from a military court or you received non-judicial punishment from military authorities.

Have you been convicted of any act(s) involving domestic violence?
