Description

San Mateo Medical Center's Willow Clinic and Fair Oaks Family Health Center has 2 full-time Nurse Case Manager positions available in the Same Day Clinics. These are urgent care and primary care clinics serving the East Palo Alto, Menlo Park, and Redwood City communities. The Nurse Case Manager is responsible for direct and indirect delivery of patient care for a varied patient population through the use of the nursing process and maintenance of the standards of professional nursing practice. The Nurse Case Manager promotes compliance and support of policies and procedures, knows their legal responsibilities and licensure requirements, participates in evaluation of quality assurance issues by identifying problems and proposing solutions; assists in assigned data gathering; participates in educational programs to maintain and develop nursing skills.

Examples Of Duties

Duties may include, but are not limited to the following: Performs nursing care following prescribe rules, policies, protocol and procedures; establishes and implements immediate nursing care plans for each patient through assessment, development, activation and evaluation; formulates case management plans to appropriately address patient care needs based on order forms, and consultation with physicians, professional/paraprofessional nursing staff, patients, families and caregivers; optimizes patient health care outcomes by providing appropriate education, referral(s) and follow-up; organizes, assigns and supervises duties and responsibilities of other staff members as a team leader; participates in non-clinical assignments which may include maintaining records, data collection, preparing reports and/or serving on special committees; responsibilities for orienting and reaching new staff members of the nursing team and students; be able to triage and screen on site as well as phone triage; and perform related duties as assigned.

Qualifications

Experience and Education: Current CA RN license. Previous experience in Case Management and Triage Nursing is preferred.

Depending on the number of applicants, an application appraisal of education and experience may be used in place of other examinations or a screening committee may select those applicants whose education and/or experience appear to best meet the needs of the position. Paid Time Off

Holidays

There are 12 holidays per year. There are 11 fixed holidays:

· New Year's Day – January 1st

· Martin Luther King Jr's Birthday – the third Monday in January

· President’s Day – the third Monday in February

· Memorial Day – the last Monday in May

· Independence Day – July 4th

· Labor Day – the first Monday in September

· Columbus Day – the second Monday in October

· Veterans Day – November 11th

· Thanksgiving Day – the fourth Thursday in November

· The Friday after Thanksgiving Day

· Christmas Day – December 25th

Additionally, on February 12th of each year every employee receives a floating holiday that can be used at any time in the future.

Vacation Time

Employees receive vacation time based on years of service as follows:

0 - 5 years 13 days per year

5 - 10 years 16 days per year

10 - 15 years 19 days per year

15 - 20 years 21 days per year

20 - 25 years 22 days per year

25+ years 24 days per year

You can accrue up to two times your annual accrual rate.

Sick Leave

Employees receive 12 days of sick leave per year. There is no limit to the amount of sick leave you can accrue.

Insurance

The County offers its employees one of the finest and most comprehensive health benefits programs available in the workplace today.

Health Coverage

The County offers a choice of Aetna, Blue Shield or Kaiser Health Coverage. County and employee contributions depend on the plan selected and the number of dependents.

Domestic partners (either sex) are covered by the health plans. Additionally, young adult dependents can be covered up to 30 years of age.

Dental Coverage

The County offers a self funded plan through Blue Shield/Dental Benefits Provider and Delta Dental coverage. The County pays 90% of the insurance premium for the coverage.

Both plans provide coverage for domestic partners (either sex) and for young adult dependents up to 30 years of age.

Vision Insurance

The County offers fully paid vision insurance to all employees. The plan provides coverage for domestic partners (either sex) and for young adult dependents to the age of 30.

Life Insurance

The County provides $9,000 in term life insurance coverage.

Employees may also purchase additional term life insurance to a maximum of $250,000 for the employee, $125,000 for spouses and $10,000 for dependents.

Pension and Savings Programs

Retirement

The County offers two retirement plan options for new employees. Benefits vary by plan selected, years of service and age at retirement.

Employee contributions to retirement are pre-tax.

Social Security

Employees are covered by the federal Social Security system and earn benefits for retirement based on salary and time worked.

Deferred Compensation

The County offers Deferred Compensation benefits through Hartford and National Deferred for employees to voluntarily set aside a portion of their salary to be invested for payment at a later date upon termination or retirement. Neither the deferred amount nor earnings on the investments are subject to current federal or state income taxes. Taxes become payable when deferred income plus earnings are distributed to you, presumably during retirement when you are in a lower income tax bracket. Employees may enroll at any time during the year.

Credit Union

Employees are eligible to join the County Credit Union which offers savings options, loans and a variety of banking services.

Personal and Family

The County is committed to assisting you in providing for your family and dependents. A variety of programs are available to you.

Child Care

The County has a child care center in Redwood City and co-sponsors a child care center in Daly City. First access to the Redwood City center is for County employees. Scholarships are offered at both centers.

Additionally, the County has a child care referral service.

Dependent Care

The County offers a dependent care assistance plan - DCAP - called Care$hares. At your choice Care$hares gives you the opportunity to pay for your eligible dependent care expenses with your pre-tax dollars.

Employee Assistance Program

The Employee Assistance Program offers access to confidential counseling and referral services to assist you or members of your family in handling problems related to work, personal relationships, finances, drug and alcohol problems, or other life concerns. Eligible family members include spouse, domestic partner and young adult dependents to age 30.

Catastrophic Leave Program

The Catastrophic Leave Policy assists employees who have exhausted paid time credits due to a serious or catastrophic illness, injury or condition of the employee or his/her extended family. This program allows other employees to make grants of time to that employee so that s/he can remain in a paid status for a longer period of time, thus partially ameliorating the financial impact of the illness, injury or condition.

Short Term Disability Program

The County provides employees either State Disability Insurance coverage for employees with benefits up to $490 weekly.

Long Term Disability Program

The County pays for a Long Term Disability Insurance program. The LTD program will pay up to two-thirds of your salary, to a maximum of $2,400 per month, minus any amount paid by another group disability plan.

Tuition Reimbursement

The County provides a tuition reimbursement program that will repay employees for up to $438 per course successfully completed.

Voluntary Time Off

The County provides flexible working hours for County employees through the Voluntary Time Office program. Employees can reduce their time at work by up to 20% while maintaining full time status for most County benefits. Time off can be used to reduce the work day or work week or can be taken in longer blocks of time.

Telecommuting
